Care Coordination

Social workers provide long term care management assistance to at-risk
members age 65 and older. Assistance may include:

• Information and referrals to community services and resources such as home delivered meals, transportation, help at home, financial programs and services
• Counseling to address health and social changes affecting daily living
• Planning for future care needs

Dual Complete Program (formerly Special Needs Program)

Licensed Clinical Social Workers and RN Case Managers provide assistance to adult Kaiser Permanente Senior Advantage Medicare / Medi-Cal Plan Members. Assistance includes:

• Annual assessment
• Individualized care plan
• Interdisciplinary care team consultation (MD, RN, LCSW, Pharmacist and Behavioral Health Liaison)
• Coordination of benefits and care:
  • telephone call after discharge from the emergency room, hospital or nursing home
  • help with getting Medi-Cal covered services and community services
